"I wish to file a formal complaint against a case worker at Madison Hospital for their repeated actions of reporting our family to the ********** of *************** (DHR) every time I request their assistance in contacting the insurance company to arrange transportation for my terminally ill mother. This situation has become distressing as it appears that they are misusing the system by unjustly reporting us for neglect when, in fact, I simply lack the means to provide transportation for my mother." ...
