On September 17th, 2023, Utah Division of Professional and Occupational Licensing (DOPL) revoked the license to practice as a pharmacy for HRX Pharmacy, LLC with a stay of
enforcement on the revocation. Said license was placed on probation for four years
with conditions. Respondent was also ordered to pay a $22,650 fine due to violations noted during a May 2022 pharmacy inspection. For more information go to
