      Zero stars, saw a provider x2 and then they were not working there anymore. The staff then said I need to do a 2nd intake appointment ($400) even though had just had one 2 months earlier. Called billing, they stated I only needed a ($200) appointment as had already just had an intake appointment 2 months prior. I asked for this in writing. Almost 1 month later, I received a letter in the mail stating the appointment would be another ($400) intake appointment. Huge rip off. Do not recommend.
