"My daughter had all 4 wisdom teeth extracted, and 5 fillings placed, was placed on IV sedation, to have all the work done at one time. They made her sign a waiver to pay out of pocket for the IV sedation portion only, instead of submitting to our insurance, the date of service on the form was wrong." ... Read full review
